Attendee Acknowledgment and Cancellation Information

Communications - OSSGA will be using QR code technology on name badges to allow for the sharing of information between attendees and exhibitors. Information including name, email, phone, address will be included.  Each attendee controls who they give permission to for scanning, In addition, OSSGA will utilize the contact information on this form to provide event news and updates.

By registering, you and the people you register acknowledge that an inherent risk of exposure to COVID-19 and other airborne illinesses exists in any public place where people are present.  By attending the OSSGA Event, you and any attendees voluntarily assume all risks related to exposure to COVID-19 and other illnesses and agree not to hold OSSGA, staff, contractors, venues, exhibitors, speakers, or any of their affiliates, directors, officers, employees, contractors, or volunteers liable for any illness or injury.  OSSGA has the right to shift the event to a virtual format if the government limits event/meeting capacities or at its discretion. 

Cancellations must be requested in writing via email, fax, or Canada Post to the Ontario Stone, Sand & Gravel Association. Cancellations received by January 10, 2025, will receive a full refund minus a $500 administrative fee. Cancellations received January 11 - February 1, 2025, will receive a 50% refund.  There will be absolutely no refunds after February 1, 2025. If an attendee cannot attend, payment is still due, but a substitution can be made. Credit will not be given for late arrival or unattended events. Registration is for one individual and cannot be shared.
